No approval appears on the phone
Check the setup mode, installed integration, account connection, and notification path.
Start by tracing one request in the hook debug log. It separates an agent-hook problem from an API, policy, account, or iPhone delivery problem.
If an action appears but its name is Unknown Tool, the delivery path is working. Follow A tool appears as Unknown Tool instead.
Then work through these checks in order:
- Run
npx agentapprove statusand confirm you are using Approval Mode. - Confirm that the coding agent you are running appears in the installed integration list.
- Keep the iOS app open and send another low-risk test from a new interactive agent session.
- Confirm that the phone and development computer both have network access.
- Check whether an existing remembered rule handled the command without asking you.
- Run
npx agentapprove installif the integration is missing or its files need to be refreshed.
Check whether the agent still needs to enable the hooks
Some agents list the hooks they find and leave each one switched off until you approve it. The installer writes the configuration, the agent reads it, and nothing runs until you turn the hooks on. An installation in this state reports its files as present and still sends nothing.
OpenAI Codex and the ChatGPT app work this way. See Activate the new integration for the steps.
If your agent is not listed here, check whether it has its own hook or extension permission screen before assuming the installation failed.
If events appear only while the app is open, check iOS notification permission for Agent Approve. If nothing appears, collect a safe support report and contact Support.
